What is a coeliac test?

Coeliac disease is an autoimmune condition in which the immune system reacts to gluten, a protein found in wheat, barley and rye. This reaction damages the lining of the small intestine, making it harder for your body to absorb important nutrients.

A coeliac blood test is usually the first step in investigating coeliac disease. It looks for antibodies that may indicate your immune system is reacting abnormally to gluten. If your blood test is positive, further investigations, such as an endoscopy and biopsy, may be recommended to confirm the diagnosis.

Symptoms to look out for

When might you need a coeliac test?

Coeliac disease does not always cause obvious digestive symptoms, and they vary from person to person. A coeliac test may be recommended if you are experiencing:

  • Persistent diarrhoea, bloating or abdominal pain
  • Constipation or a change in bowel habits
  • Unexplained weight loss
  • Ongoing tiredness or fatigue
  • Iron deficiency anaemia without an obvious cause
  • Frequent mouth ulcers
  • A close family history of coeliac disease
  • Poor growth or delayed development in children
  • An itchy, blistering skin rash (dermatitis herpetiformis)

Many of these symptoms can also occur with other conditions, such as irritable bowel syndrome (IBS), thyroid disorders, vitamin deficiencies or inflammatory bowel disease. A GP assessment helps determine whether a coeliac blood test is the most appropriate investigation.

What does the coeliac blood test measure?

A coeliac blood test looks for antibodies that may indicate your immune system is reacting abnormally to gluten. Depending on your symptoms and medical history, your investigations may include:

Tissue Transglutaminase Antibodies (tTG-IgA)

The main screening test for coeliac disease, looking for antibodies produced when the immune system reacts to gluten.

Total IgA

Checked alongside tTG-IgA to make sure your coeliac blood test results can be interpreted accurately.

Important before your test

Continue eating gluten before your coeliac test

One of the most important things to remember before a coeliac blood test is not to stop eating gluten. The test relies on detecting antibodies that develop when someone with coeliac disease continues to eat gluten.

Unless Dr Faizal advises otherwise, keep to your normal gluten-containing diet before your blood test. There is no need to fast beforehand, so you can eat and drink as normal on the day of your appointment.

After your results

What happens after your results?

Once your results are available, Dr Faizal will explain what they mean and discuss the next steps if needed. Depending on your results, he may recommend:

  • Reassurance if your results are within the expected range
  • Repeat blood tests if further monitoring is required
  • Additional investigations if another underlying condition is suspected
  • Referral to a gastroenterologist if further assessment, such as an endoscopy and biopsy, is appropriate
